Board index All times are UTC - 6 hours Legionnaires Sancti Michaelis - Confidentiality Agreement CONFIDENTIALITY AGREEMENT: Members of the Legionnaires Sancti Michaelis-€, are required to read and to agree to the Confidentiliaty Agressment stated below. press the I agree to these terms button if you agree, otherwise, press I do not agree to these terms. IMPORTANT: Please read all sections carefully as you are morally and legally accounible to know and to adhere to this document. If you have any questions, please ask before proceeding. I. Confidentiality of Client Information I understand and acknowledge that: services provided to clients by the St. Padre Pio Center for Deliverance Counseling (SPCDC) or of the Legionnaires of St. Michael (LSM) are private and confidential; to enable such services to be performed, clients provide personal information with the expectation that it will be kept confidential and used only by authorized persons as necessary; all personally identifiable information provided by clients or regarding counseling services provided to clients, in whatever form such information may exist, including oral, written, printed, photographic and electronic formats (collectively, the Confidential Information-€) is strictly confidential and is protected by applicable laws and SPCDC Standards of Ethics and Practices and LSM regulations that prohibit its unauthorized use or disclosure; and in the course of my affiliation with LSM and SPCDC I may be given access to certain Confidential Information. II. Access, Use, and Disclosure I agree that, except as authorized in connection with my assigned duties as defined by the LSM Command Staff, I will not at any time use, access or disclose any Confidential Information to any person (including but not limited to co-workers, friends and family members). I understand and agree that this obligation remains in full force legally and morally during the entire term of my affiliation with SPCDC and continues in effect in perpetuity even after such affiliation terminates. III. Confidentiality of LSM and SPCDC Affairs I agree to keep in confidence and will not at any time use, access or disclose, to any unauthorized person (including but not limited any unauthorized LSM or SPCDC volunteers or staff, friends and family members), all information that I may become privy to concerning the internal affairs, processes, procedures, and any other non-public information in whatever forms such information may exist, including oral, written, printed, photographic and electronic formats, of the Legionniares of St. Michael and the St. Padre Pio Center for Deliverance Counseling, its members, employees, volunteers, benefactors, and contributors, unless given specific written permission otherwise, or as required by Law. IV. Return of Confidential Materials Upon the termination of my affiliation with LSM or SPCDC for any reason, or at any other time upon request, I agree to promptly return to LSM/SPCDC all copies of business, administrative, and client information, and any other LSM/SPCDC property, that is in my possession or control (including all printed and electronic copies). V. Periodic Certification I understand that I may be required to periodically certify that I have complied in all respects with this Agreement, and I agree to so certify when requested. VI. Remedies I understand and acknowledge that: the restrictions and obligations I have accepted under this Agreement are reasonable and necessary in order to protect the interests of clients, the Legionnaires of St. Michael, and the St. Padre Pio Center for Deliverance Counseling; and my failure to comply with this Agreement in any respect could cause irreparable harm to clients, the Legionnaires of St. Michael or the St. Padre Pio Center for Deliverance Counseling for which there may be no adequate legal remedy. I therefore understand that the Legionnaires of St. Michael and the St. Padre Pio Center for Deliverance Counseling may prevent me from violating this Agreement by any legal means available, in addition to disciplinary measures, which may result in sanctions in accordance with applicable policies for members of the Legionnaires of St. Michael or the staff of the St. Padre Pio Center for Deliverance Counseling. VII. Terms of this Agreement I understand and agree that this obligation remains in full force by law and by moral imperitive during the entire term of my affiliation with LSM/SPCDC and continues in effect in perpetuity even after such affiliation terminates.
